No Surprises Act and Good Faith Estimate

At Destined for Greatness Counseling LLC, we are committed to transparency and affordability in mental health care. As part of this commitment, we adhere to the No Surprises Act, which protects clients from unexpected medical bills and ensures clarity about the cost of services.

Your Rights Under the No Surprises Act

  • Transparency in Billing: You have the right to receive a Good Faith Estimate (GFE) explaining the cost of services before your first appointment or at any time you request it.

  • Clear Communication: We aim to provide upfront information about your treatment costs so you can make informed decisions about your care.

  • Protection from Surprise Charges: If you are uninsured or prefer to self-pay, you are entitled to a written estimate of what you can expect to pay for services.

Requesting a Good Faith Estimate

To receive your Good Faith Estimate, please contact our office at least 48 hours before your scheduled session. You can also request an estimate at the time of booking. The estimate will include the cost of your planned services, and we’ll provide it in writing to ensure full transparency.

Sliding Scale Fee

We understand that financial situations vary, and we strive to make quality mental health care accessible to everyone. Our sliding scale fee is designed to accommodate clients based on their income levels and financial needs.

Determining Eligibility

We base our sliding scale fees on the federal poverty guidelines and consider:

  • Household income

  • Number of dependents

  • Other financial obligations

Sliding Scale Fee Ranges by State

Texas, Louisiana, and Georgia Residents:

  • Fees are adjusted based on a percentage of the federal poverty income line for your household size.

  • Supporting documentation such as pay stubs or tax returns may be requested to determine eligibility.

Fee Structure Example:

Household Income Level (Percentage of Federal Poverty Line)/ Session Fee

Up to 150% -$50 per session

151% - 200%- $75 per session

201% - 300%- $100 per session

Above 300%- Standard rates apply
